Breast milk changed colour? Here’s what yellow, green, pink and brown shades may mean

For a new mother, seeing breast milk that looks yellow one day, almost bluish the next or faintly pink in the bottle can be startling. Is the milk still safe?The reassuring answer is that breast milk does not have one fixed colour. Its appearance can change with the stage of lactation, the amount of fat in the milk, what the mother eats, certain medicines and, sometimes, something happening in the breast itself.

Science says that human milk is a remarkably dynamic substance rather than a uniform liquid whose composition stays the same from one feed to the next. So, what do those different shades actually mean? Here’s what you need to know

Yellow or golden

The first milk a mother produces is colostrum, and it is commonly yellowish or golden. A systematic review, published in the Nutrition Reviews, analysed data from 65 studies and found that several carotenoids were present at higher concentrations in colostrum than in mature human milk.In other words, that golden-yellow appearance is part of the biology of early lactation, not a sign that something is wrong.

Green

Green-tinted milk can look dramatic, but colour changes can occur after consuming strongly coloured foods or drinks, including foods containing green pigments or food colouring. TheAustralian Breastfeeding Associationnotes that foods, drinks and food dyes can influence the appearance of breast milk, while some medications can also alter its colour.

Pink, red or rusty

Pink or reddish milk is often the colour that alarms parents most- and understandably so. Sometimes the explanation is relatively simple: a small amount of blood has entered the milk. Cracked or injured nipples can cause this, particularly during the early weeks when breastfeeding is still being established.There is another unusual but generally benign explanation called Rusty Pipe Syndrome. It causes blood to appear in breast milk, giving it a pink, red, brown or rust-like appearance. It is usually painless and tends to resolve on its own.Importantly, the presence of blood does not automatically mean breastfeeding has to stop. The American Academy of Pediatrics lists mastitis, for example, as a condition in which breastfeeding is not contraindicated.Note: If your breast milk remains blood-tinged for a few days or more, you should probably contact a health care provider, as pink or red breast milk can indicate other issues.

Dark brown or black milk

This is much less common and should not simply be dismissed as another harmless colour variation. TheNIH’s LactMed database, which compiles evidence on drugs and breastfeeding, documents rare cases of black breast milk associated with minocycline.

When should parents call a doctor?

Colour alone is often not the most useful measure of whether breast milk is healthy. But a new, persistent or unexplained change deserves attention, particularly if it comes with breast pain, swelling, redness, fever, pus, a breast lump, significant nipple injury or the mother feeling unwell.The same applies when milk repeatedly appears bloody, especially if there is no obvious nipple trauma or if the change is coming from only one breast. Rusty Pipe Syndrome is generally benign, but persistent bleeding should not simply be assumed to be RPS.And there is another important distinction: colour and safety are not the same thing. Even perfectly normal-looking expressed milk can become unsafe if it has been improperly stored or handled. The bigger picture says that the color, texture, and composition of breastmilk can shift as the baby grows, the mother moves through different stages of lactation, and even as a feed progresses. What matters far more is that any persistent or concerning change should be discussed with a paediatrician, lactation consultant or other qualified healthcare professional.
